The Department of Mathematical Sciences, together with the Office of the
Registrar, is continuing to utilize the system of prerequisite checks and
placement on all students registering for the following Mathematics courses:
MATH M001, 001, 110, 111, 130, 131, 132, 136, 153, 154, 159, 165, 166, 171,
221, 222, 231, 232 and MATH M118 and M119.
Please note: At the very bottom of the form, please be sure to select the person in your department/unit who can complete the math authorization. If no one in your department/unit has this ability, please select "Andrea Brian", to send the authorization request to Andrea Brian, academic advisor in the math department.
